WAFDA Club Affiliation Application Formhot!

Application form for clubs wishing for formally affiliate with the WAFDA.
Applications must be completed and submitted with supporting documentation to the WAFDA. Applications will be assessed by the WAFDA Clubs Officer and if all is in order will be presented to the WAFDA Committee for approval.
